Its initiation was made possible thanks to the valuable support from <strong><a href=\"https:\/\/www.repower.com\/it\">REPOWER<\/a><\/strong>.<\/p>\n\n\n\n<p>The study, coordinated by <strong>Dr. Andrea Arighi<\/strong>, aims to use these vesicles as <strong>biomarkers<\/strong> to detect the disease in its early stages, monitor its progression, and, in the future, enable more targeted interventions for a condition as widespread as it is debilitating.<\/p>\n\n\n\n<p><strong>Alzheimer\u2019s disease<\/strong> is indeed one of the most painful and widespread challenges of our time (Scheltens et al., 2021). It affects millions of people worldwide, erasing <strong>memories, relationships, and identity<\/strong> \u2014 and leaves a deep mark on those who remain close, including families, caregivers, and society as a whole, which confronts the fragility of memory and time every day.<\/p>\n\n\n\n<p>Yet, behind this loss, research continues to seek new paths to restore <strong>hope<\/strong>, to understand what happens in the brain\u2019s invisible circuits, and to find ways to halt, or at least slow, the process that leads to the disease.<\/p>\n\n\n\n<p>Today, that hope passes through something infinitely small: <strong>extracellular vesicles<\/strong>, microscopic particles released by every cell in our body to communicate with others (Sigdel Set al., 2023).<\/p>\n\n\n\n<p>Within each vesicle lie fragments of information \u2014 <strong>proteins, lipids, RNA<\/strong> \u2014 reflecting the state of the cell from which they originate. In the <strong>blood<\/strong> and even in <strong>saliva<\/strong>, these vesicles carry messages that reach the <strong>brain<\/strong> and originate from it: tiny traces of <strong>neuronal activity<\/strong>, signs of <strong>inflammation<\/strong>, or <strong>cellular stress<\/strong>.<\/p>\n\n\n\n<p>Analyzing them allows for a sort of \u201c<strong>liquid brain biopsy<\/strong>,\u201d capable of revealing what is happening in the nervous system without invasive procedures.<\/p>\n\n\n\n<p>A single drop of <strong>blood<\/strong> or <strong>saliva<\/strong> can therefore contain valuable clues about the <strong>brain\u2019s health<\/strong> \u2014 the <strong>brain in a drop<\/strong>, so to speak.<\/p>\n\n\n\n<p>The project, launched within the &#8220;<strong><strong>Centro Dino Ferrari<\/strong><\/strong>&#8221; at the <strong>Fondazione IRCCS Ca\u2019 Granda Ospedale Maggiore Policlinico of Milan<\/strong>, explores this new <strong>biological language<\/strong>.<\/p>\n\n\n\n<p>The goal is to transform <strong>extracellular vesicles<\/strong> into <strong>diagnostic and therapeutic keys<\/strong> for <strong>Alzheimer\u2019s disease<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Identify early biomarkers<\/strong> capable of signaling the onset of the disease before symptoms appear;<\/li>\n\n\n\n<li><strong>Determine which patients<\/strong> with initial cognitive difficulties are at higher risk of progressing to full-blown Alzheimer\u2019s;<\/li>\n\n\n\n<li><strong>Use the vesicles themselves as natural drug carriers<\/strong>, to protect neurons and modulate <strong>inflammatory responses<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p>To make all this possible, the laboratory requires a new <strong>extracellular vesicle isolation system<\/strong>, a high-precision instrument that allows these tiny particles to be separated and analyzed in a <strong>reproducible and reliable<\/strong> way.<\/p>\n\n\n\n<p>A significant portion of the budget \u2014 <strong>\u20ac15,000<\/strong> \u2014 will be allocated to this investment, which will accelerate the identification of vesicles, paving the way for <strong>future studies<\/strong> and <strong>large-scale clinical collaborations<\/strong>.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"1024\" src=\"https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-1024x1024.jpg\" alt=\"\" class=\"wp-image-2763\" srcset=\"https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-1024x1024.jpg 1024w, https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-300x300.jpg 300w, https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-150x150.jpg 150w, https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-768x768.jpg 768w, https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-1536x1536.jpg 1536w, https:\/\/www.centrodinoferrari.com\/wp-content\/uploads\/2025\/12\/Fenoglio-Serpente1-2048x2048.jpg 2048w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p>Every step forward in this direction represents an <strong>act of hope<\/strong>: for those living with the disease, for those who accompany them every day, and for all who believe in a <strong>science capable of restoring meaning and memory<\/strong>.<\/p>\n\n\n\n<p>Inside a <strong>drop<\/strong>, today, research tries to listen to the <strong>voice of the brain<\/strong> \u2014 a voice that could, one day, teach us how to <strong>heal it<\/strong>.<\/p>\n\n\n\n<p>Thanks to this project, the lives of many people awaiting answers could truly change. A caregiver shares their testimony:<\/p>\n\n\n\n<p><em>&#8220;I want to sincerely thank <strong>Dr. Andrea Arighi<\/strong> and his team (especially the neurologists and neuropsychologists) at the <strong>Dino Ferrari Center<\/strong>. I am a physician, but in this case I am writing as a family member of someone very dear to me, who has been under Dr. Arighi and his team\u2019s care for years. In a journey often full of uncertainty, fear, and even suffering, from the very first meeting with the colleagues, I experienced the certainty of being followed with the <strong>highest level of scientific care available today<\/strong>, together with great <strong>humanity<\/strong> \u2014 never sentimental, but always welcoming. For a caregiver, this means not feeling alone or, worse, desperate. I can say that I have learned a lot even as a doctor from this journey with the <strong>&#8220;<strong><strong>Centro Dino Ferrari<\/strong><\/strong><\/strong>&#8220;. The presence of this team does not remove all the weight that the disease brings, but it makes you feel <strong>accompanied and supported<\/strong> along the way.
